DPOA and Advanced Directives with ESH SS

Understanding advanced directives is an important part of planning for your future healthcare. Join our social worker for an informative session on advanced directives, where we will guide you through the process of making decisions about your medical care in the event you are unable to communicate them yourself. Additionally, our social worker, who is a notary, will discuss Medical Durable Power of Attorney and assist you in completing one after the session, if you wish.
