Mini Clinic registration is still going on but only a few more weeks to register!! There are several ways to pick up and turn a form:
pick one up AND turn them in to your school’s office
pick one up AND turn them in to the community center (in a sealed envelope)
e-mail us your form and call the AD office to pay over the phone: [email protected]
